Participants who are under the age of majority for the region of the event should still Pre-Register here and now, but make sure you do three things: 1. Indicate that you ARE under the age of majority on this form. 2. Print the Underage Waiver found at http://www.thrilltheworldnyc.com/childwaiver.pdf 3. Bring a signed form to the event on October 27th, 2012. (Signed by Parent/ Legal Guardian) If you are underage and do not bring a signed form you may not be allowed to participate. RESPONSIBILITY WAIVER I, the Participant, (refers to “Participant,” “I”, “My”) acknowledge that there are risks associated with participating in physical activities and dance. My participation is completely voluntary and by participating I acknowledge that I am assuming all risks of accident, injury, illness and death which may occur during or as a result of participating in this event. I, the Participant, agree on my own behalf (and on behalf of my personal representatives, heirs, estate trustees or assigns) to a) release, indemnify and discharge The Event Organizers (“The Event Organizers,” and “The Event” refers to Ines Markeljevic, Thrill The World, Inessens, and the local event organizers; Amada Anderson, volunteers, venue, city, anyone else involved in the local event, sponsors and partners of the event, including respective owners, organizers, officers, directors, agents, employees, independent contractors, volunteers, or legal representatives) from any and all claims and causes of action (known or unknown) which may be arising out of The Event Organizers’ negligence, including negligence of staff, agents or representatives of the event or those of the venue, and b) to indemnify and save The Event Organizers’ harmless from any and all claims of causes of action (known or unknown) brought against The Event Organizers by any party arising out of My actions, including My negligence while at The Event. The Event Organizers are not responsible for any damage to, loss or theft of personal articles or property during the event. PHOTO/IMAGE WAIVER I, the Participant, consent for my image, and likeness, to be photographed, filmed or videotaped on October 27th, 2012, in relation to The Event and is the property of, Ines Markeljevic, Thrill The World, and Inessens. GROUNDS FOR DISMISSAL “Dismissal” means that the Participant must cease participation in The Event and leave the venue premises immediately. In an effort to maintain a safe environment for all participants, staff, employees, volunteers, etc. and media present at The Event, The Event Organizers reserve the right to dismiss and remove a participant if the participant a) fails to follow the rules and regulations, b) causes a nuisance or disturbance, c) commits any illegal or immoral acts, or d) if The Event Organizers feel the Participant’s actions may endanger the participant or others. The Event Organizers reserve the right to dismiss the Participant without cause. Use of alcohol or drugs before or during the event is prohibited and will result in dismissal from The Event and the premises. Participants in possession of alcohol or drugs before, during, or while on the property of venue, will be dismissed and removed from The Event and the premises. Legal charges may be pressed. DONATIONS TO THRILL THE WORLD NYC: Please note that we are an official non-profit organization, this event is run by Michael Jackson fans and is no way endorsed by Sony Music Entertainment. All Thriller Classes are $20 per Thriller class and/or $10 and a pair of children's pajamas or a New Children's Book for our chosen charity The Pajama Program). Payments made thru Eventbrite will go directly to the event itself and the event manager for immediate reinbursements (Park & Soung Permits, Batteries for Ipod Player, Deposits for venues, etc.) via paypal. Make sure you purchase your ticket from above.  There are two options for each class, please remember other studios charge $20 for a one hour class our class is 2 hours! Our class is from 3pm-5pm and has a guarantee that you will learn the dance;)  a.)$20  OR b.)$10 and a donation to The Pajama Program (childrens book or pajamas) 2. Make sure you have signed up for our mailing list by Clicking HERE. 3. If you are interested in FREE THRILLER CLASSES you should join our internship program. It's easy. Just email Amada and say you want to help. Her email is thrilltheworldnyc@gmail.com. 2 hours of volunteer work= ONE FREE CLASS. No need to sign up for the class she will simply add you to the list on the date you plan on attending the class. Volunteering/Internship can be anything from recruiting people via passing out flyers or administrative work. (Making phone calls to schools or organizations on behalf of TTWNYC). TTWNYC THRILLER CLASSES will be from 3pm-5pm in Central Park. Meeting at West 59th/Central Park West and 7th Avenue unless otherwise noted.
